Your Coverage Agreement with Patients

PATIENT RESPONSIBILITIES
- Complete continuing care (hygiene) visits as prescribed – at least every 8 months (6 months with a 2 month grace period)
- Keep account current
- Notify dentist/practice of any issue within a reasonable time after discover – not to exceed 7 days
- Patient’s failure to provide timely notification, or failure to appear for a scheduled appointment to address an issue is grounds for voiding coverage
- Rescheduling is permitted if required by patient or practice
PRACTICE RESPONSIBILITIES
- Provide maintenance services (repair, replace, or re-treat) at no charge to the patient, whether or not you are eligible to be reimbursed by Dental Warranty via a claim, subject to the following limitations and exceptions:
- Value of warranty is limited to the original treatment cost covered, as reflected in the coverage’s remaining value
- Coverage may be deemed void by practice if patient fails to keep up with their responsibilities
- Services provided beyond remaining value of coverage may be billed to the patient
- Services required due to issues/failure within first 90 days without external trauma (First 90 Days Rule) may not be billed to the patient – no claim is filed and coverage continues for remaining term
FOLLOWS YOUR SMILE BENEFIT
- If your patient has moved away or is traveling such that they cannot reasonably return to you for care, the coverage may be redeemed with any dentist with your consent
- Your agreement with patients states a specific distance requirement of 100 or more miles. This is not a requirement in place to limit patients from utilizing their coverage, rather a parameter to discourage patients from leaving unnecessarily to see another practice within your local area.
WARRANTY CANCELLATION AND REFUNDS
- If a patient requests cancellation and a refund, your practice may cancel if within 7 months of the covered treatment date.
- Upon cancellation, Dental Warranty will refund the amount originally billed to the practice.
- You are responsible to refund the amount paid by the patient to your practice for coverage.
- After 7 months from the treatment date, no refunds will be available from Dental Warranty to the practice.
- It is best practice to obtain a signed consent or request to cancel coverage from your patient in order to maintain clear communication and expectations
