Use the following guidelines as you offer smile protection to patients.
PATIENT ELIGIBILITY
- Patient is free from pre-existing medical conditions that would impact treatment success
TREATMENT / APPLIANCE ELIGIBILITY
- Treatment is not temporary, experimental, untested, or unproven
- Treatment is not an alternative to more permanent and recommended plan for the patient
- Dentist is free from reasonable doubt that treatment will last at least the term length of coverage
- Procedure is listed under Eligible Procedures, as provided by Dental Warranty
SPECIFIC PROCEDURES & APPLIANCES NOT ELIGIBLE FOR COVERAGE:
- Snap On Smile
- Clear tray aligners
- Including but not limited to:
- Invisalign
- ClearCorrect (Straumann)
- SureSmile Clear Aligners (previously MTM Clear Aligners – Dentsply Sirona)
- 3M Clarity Aligners
- Including but not limited to:
- Thermoplastic retainers
- Including but not limited to:
- Vivera (Invisalign retainer)
- Essix Plastics material
- Zendura material
- Including but not limited to:
- Resin-bonded bridges (‘Maryland Bridge’)
- Temporary removable partial dentures (‘flippers’)
PRIOR COVERAGE, 2X LIMIT
- Treatment (same tooth, same procedure) has not already failed 2 times under coverage
- Coverage may be issued up to 2 times per instance of the same tooth and procedure
- a 3rd warranty on same tooth, same procedure is not offered to the patient
- coverage may be offered if procedure/treatment is considered next step – the treatment is a different procedure
- As a reminder, each warranty holds a value equal to the original treatment fee of the covered procedure. When fully redeemed (entire value used for replacement), you may offer a new warranty on the re-treatment. Your patient doesn’t pay again for the procedure itself, only their fee for the new protection plan.
