Begin typing the patient’s name and select from the dropdown matches.

If no matches appear, you will need to add the patient by clicking the ‘Add Patient’ button.

Select the Dentist who performed the treatment being covered.

Enter the ‘Treatment Date’ by clicking the calendar icon to the right of the field.

We have intelligence built into the Tooth/Range field – you can now enter in a single tooth, range of teeth, appliance or arch and we will automatically recognize and apply your input correctly.
For example you can type in 5 for a single tooth, 2-5 for a range of teeth, upper for the upper arch, and sleep for a sleep appliance. Simply type in what you need and we’ll take care of the rest.

Enter/select the ‘Procedure Code’ being covered, surfaces if applicable, and the ‘Treatment Value’.

To add another treatment being covered, simply click the ‘Add Another Treatment’ button, and repeat the steps above for each treatment/coverage to be registered.
